5/31/2023 EnchantaVaughn Approved 1.Behavior: QMHPA greeted Kaila upon arrival to the home setting. Upon arrival to the home setting, Kaila appeared to be in a positive mood evident by her overall disposition. QMHPA inquired about her morning. Kaila discussed with QMHPA that she was doing well on this morning. Kaila did not present any symptoms of S/I or H/I at the time of the session. 2.Behavior: Kaila at times was observed displaying hopeful behaviors and engaged in the session without redirection to focus on session. 3.Behavior: Kaila appeared to become agitated when discussing ways to implement coping skills as she reports it is sometimes hard for her because of her anger. 1.Intervention: QMHPA engaged Kaila in a discussion surrounding her mental state. QMHPA discussed with Kaila different triggers that are associated with depression. QMHPA informed Kaila that the purpose of this discussion was to assist her with being able to understand the symptoms her body will endure physically and mentally. QMHPA encouraged Kaila to open up more and reduce isolative behaviors. QMHPA informed Kaila of the activity was to assist Kaila with identifying her triggers at times and recognizing impulsive behaviors. QMHPA assisted Kaila in recognizing her behaviors through role play. QMHPA questioned Kaila about her daily activities to better understand her isolative behaviors during the day. QMHPA engaged Kaila in a skill building activity. QMHPA informed Kaila that the purpose of this activity was to assist Kaila with identifying the importance of utilizing positive social skills. 2. Intervention: QMHPA educated Kaila that this activity was to for her identify appropriate coping skills in an effort to manage anger. QMHPA engaged Kaila in a role play activity. QMHPA informed Kaila that the purpose of this activity was for her to demonstrate appropriate coping skills during difficult situations. QMHPA prompted Kaila to utilize coping skills discussed throughout the activity. QMHPA reviewed the activity with Kaila to ensure that she understood the purpose of the activity. QMHPA challenged Kaila to identify one coping strategy. QMHPA engaged Kaila in discussion surrounding the reasons it is important to cope. QMHPA engaged Kaila in discussion surrounding emotions which are often hard to cope with. QMHPA engaged Kaila in discussion surrounding the importance of personal and community support systems. 3. Intervention: QMHPA challenged Kaila to identify a personal support system. QMHPA engaged Kaila in a skill building activity. QMHPA informed Kaila that the purpose of this activity was to for Kaila to correlate coping strategies for difficult emotions. QMHPA informed Kaila that the purpose of this activity was to assist her demonstrating the ability to utilize identified coping skills during difficult emotions by role playing. QMHPA encouraged Kaila to implement the taught coping skills into her daily life when she is depressed or experiencing feelings of anxiety which was for her to attempt to demonstrate on a daily basis as she experiences symptoms of depression, anxiety and isolation from others due to her current mental health illness. 1.Response: Kaila appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. Kaila struggled with being able to discuss with QMHPA stressors that cause her to become depressed; however, she stressed to QMHPA that she is taking on a lot of tasks that she really should not at her age that is making her depressed. Kaila indicated to QMHPA that she sometimes isolates herself from other people so that she doesn’t have to deal with other people’s attitudes. Kaila shared that when she is in the community, she feels a little better with handling depression because she is around other people. 2. Response: Kaila actively participated in the skills building activity. Kaila communicated to QMHPA triggers evident by her identifying “when people try to take advantage”. Kaila discussed with QMHPA that her family members (brother) often attempt to take advantage of her or her friends who she has met at the clinic as they want her to always do something for them. Kaila stated to QMHPA, “That’s what makes me shut down”. Kaila identified positive coping strategies to manager anger. 3. Response: During the role play activities, Kaila demonstrated openness in an effort to become familiar with ways to manager anger however struggled with utilizing such techniques during activity. Kaila identified “being sad and overwhelmed” as a difficult emotion to manage. During role play activity, Kaila demonstrated the ability to address frustration in appropriate ways. Kaila struggled with identifying community support in which she utilizes as a means to cope with daily stressors.
